“Progressions” at Sun West Choice Healthcare & RehabWe are pleased to inform you that Sun West Choice Healthcare & Rehab offers a comprehensive rehabilitation team which includes Physical, Occupational and Speech Therapy. Our specialized team provides quality services to all community residents in need of rehabilitation. A majority of the geriatric population admitted to our facility are in transition from hospital to home. These residents receive treatments up to six (6) days a week from our specialized team so that they may regain their highest quality of life. Our staff at Sun West Choice Healthcare & Rehab believe that providing professional and loving care enriches the lives of our residents as well as our own. We are composed of skeletal, soft tissue and neurological systems, and in the course of aging, problems may occur. Following a stay in the hospital, Sun West Choice Healthcare & Rehab is the ideal place for comprehensive rehabilitation before returning home. Our state and nationally certified Physical Therapist will perform an evaluation on each resident which includes history, precautions, contraindications, observation of posture and movements, and numerous tests such as muscle strength, range of motion, functional and mobilization ability.
Various conditions which might require the expertise of a Physical Therapist to assist in the rehabilitation process include: strokes, fractures, amputations, burns, spinal cord injuries, joint replacements, arthritis, and any other problems which involve the neurological or musculoskeletal systems. As well as weakening from illness or long hospitalization. The therapist, along with the resident, will plan a treatment program to meet their specific goals. Physical Therapy concentrates on maximizing the functional status of each resident through exercises and movement. The exercises are designed to improve the strength, balance and coordination; the movement focuses on improving mobility and safety. To live productively, people must be able to independently participate in the Activities of Daily Living (ADL’s) such as bathing, dressing, grooming, leisure and homemaking. For many, the everyday process of adapting to environmental demands is complicated by strokes, hip fractures, arthritis and neuromuscular diseases. Occupational Therapy can restore the greatest possible degree of functional independence by assisting an individual in attaining the highest level of productive living. Our Occupational Therapy Program at Sun West Choice Healthcare & Rehab provides services for residents with a variety of diagnoses including stroke, hip fractures, arthritis and neuromuscular diseases. Occupational Therapy Programs focus on self-care, functional mobility, home safety awareness and adaptive equipment training. Treatment is directed towards reducing the impairment which preludes independent function, for example: loss of strength, loss of selective upper extremity use, lack of adequate range of motion, decreased ability to orient oneself, attend, sequence, recall or problem solving.
